Huron Case Management strives to provide high quality case management services designed to facilitate case resolution. With all referrals, we are committed to providing the highest level of case management services and managing the process from inception to case resolution.

How to upload successfully


Submit a referral using our online forms, or download a printable PDF version of the referral form to complete and send via fax or email. Online forms, PDF downloads, and further details are below:


Online Form Instructions


  1. Fill out all of your information
  2. Click “Select Files” in the Upload File section
  3. Select the file you would like to upload.
  4. Click “Submit Referral”

Please include the following:


  • First report of injury
  • Current correct contact information for injured worker
  • Medical Records
  • Upcoming medical appointments- location, date, and time
  • Type of Employement
  • For NC jurisdiction, IC File # if available
  • For Vocational referrals- Educational and Employment history if available
  • Special Instructions
