Why Application Assistance?

At Huron, our team of qualified case managers provide a plethora of services pertaining to application assistance for individuals who are disabled, severely injured or when a serious medical condition stops them from working. There are various community resources and government aids available for such individuals that can help cover healthcare and living expenses.

The two main Social Security programs that support people living with disabilities are Social Security Disability Insurance (SSDI) and Supplemental Security Income (SSI). Nevertheless, health insurance is a necessity for anyone living in the United States due to the high costs of healthcare.

Social Security Disability Insurance pays benefits to you and certain members of your family if you are "insured," meaning that you worked long enough and paid Social Security taxes.

Supplemental Security Income pays benefits based on financial need.

Social Security Disability Insurance and Supplemental Security Income provide critical lifelines for roughly 12 million people with disabilities. The financial assistance that Disability Insurance and Supplemental Security provide is vital as it helps individuals with severe disabilities and health conditions to live independently, keep a roof over their heads and food on the table, and pay for needed, often life-sustaining medications and other basic expenses.

While these two programs are different in many ways, both are administered by the Social Security Administration and only individuals who have a disability and meet medical criteria may qualify for benefits under either program. It is advisable to apply for both SSDI and SSI as soon as one becomes disabled. They may be eligible for one and not the other.

What We  Do?

Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care and cost-effective outcomes.

Applying for programs

Now, this is the critical part. Applying for these programs is usually a lengthy, tedious and confusing process. They have rigid eligibility criteria which makes it impossible for a large number of people to avail the benefits of the programs. When you need help applying for disability benefits, you can get help with your claim from our team of experts at Huron Case Management. We will be there for you at every step of the way while filling for disability claims.

We will provide the following services:

  • Gather all the evidence necessary to prove that you’re eligible for the benefits of the social security programs.
  • Counsel you on application and documentation strategies.
  • Assistance with your disability interview or online application.
  • Represent your interests during the review and appeals stages.
  • Help understand SSA processes, disability documentation and eligibility, and denial and appeal procedures.
  • Help with availing health insurance benefits.
  • Collecting and documenting information including

1)Medical records, surgical notes, test results, treatment files, statements from co-workers and managers, your work history, and details that show any treatment you have undergone and how you responded to the treatment.

    2)Names, addresses, and phone numbers of doctors, caseworkers, hospitals, and clinics that took care of you and the dates of your visits;

3)Your most recent W-2 form or, if you were self-employed, a copy of your federal tax return.

    4)Information about your family members

Feel free to reach out to us for help in times of distress when you and your loved ones are busy dealing with the struggles associated with a severe injury or disability.
