OT Home Evaluation & Home Modification Coordination
Why OT Home Evaluation?
Occupational therapy (OT) is the use of assessment and intervention to develop, recover, or maintain the meaningful activities, or occupations, of individuals, groups, or communities.
The American Occupational Therapy Association (AOTA) defines an occupational therapist as someone who "helps people across their lifespan participate in the things they want and need to do through the therapeutic use of everyday activities (occupations)”
We strive to provide exceptional occupational therapy that can greatly help people of all ages to fully participate in the things and activities that matter to them. Our occupational therapists help the clients with many things such as:
- Enable people to live their life to its fullest by promoting health
- Help prevent—or live better with—injury, illness, or disability
- Help children with disabilities to participate in school and social life
- Help people recover from severe or traumatic injuries
- Lending support to elderly people going through physical and cognitive changes

What We Do?
Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care and cost-effective outcomes.
What makes us different
Home Assessment
Occupational therapy involves providing our clients with the tools to modify their home environments in accordance with their abilities and promote proper participation in daily life activities. Home modifications are changes made to optimise living spaces to maximise usage, safety, comfort, physical and mental well being, security, and independence.
Home Modification
Home modifications are important in allowing individuals to grow old in their home of choice, by making their home a place that’s conductive to their physical and mental well being. 77 percent of 50-plus American adults agree with the statement, “I’d really like to remain in my community for as long as possible,” according to AARP’s “Home and Community Preferences Survey,”.
